Assumption Of Risk: I, the undersigned parent/guardian, understand that participation in youth football with the Long Island Cowboys, operated under Universal Interactions Inc., involves inherent risks. These risks may include, but are not limited to, physical injury, contact with other participants, and equipment-related hazards.

I voluntarily assume all risks associated with my child’s participation.

Travel Authorization: I give permission for the Participant to travel with the Long Island Cowboys team for games, tournaments, and team-related events. Travel may include: Private vehicles driven by coaches, staff, approved volunteers, or transportation companies Chartered buses or vans. I understand that reasonable supervision will be provided, but I acknowledge that travel involves additional risks beyond the control of the organization.*

Release Of Liability: On behalf of myself and the Participant, I hereby release, waive, discharge, and hold harmless: Universal Interactions Inc., the Long Island Cowboys program, its directors, officers, coaches, volunteers, sponsors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to participation in the program or travel activities, including those caused by negligence, to the fullest extent permitted by law.*

Code Of Conduct Agreement: I understand that the Participant is expected to: Follow all team rules and coaching instructions, Demonstrate good sportsmanship and respectful behavior, Refrain from fighting, bullying, or inappropriate conduct, Adhere to travel guidelines and curfews when applicable. Failure to comply may result in disciplinary action, including suspension or removal from the program.*

Governing Law: This agreement shall be governed by and interpreted in accordance with the laws of the State of New York.*

Acknowledgement & Signature: I have carefully read this agreement, fully understand its contents, and sign it voluntarily. I understand that I am giving up substantial rights, including the right to sue.*
